Financial Assistance Interest Form
- On the 15th of each month, three families will be randomly selected to receive a grant to help ease the financial strain of therapies, adaptive equipment, home renovations, or other needs. You only need to fill this form out once. If your family is not selected this month, don’t worry! Your entry will automatically be entered into the next month’s selection.
